Periodontal Wellness Plan
$999 Perio Plan
Specially designed for patients managing gum health, this plan includes:

- 1 comprehensive exam (oral cancer screening, head & neck exam, tooth & gum exam)

- Plans are non-refundable and valid for 365 days from the purchase date.
- Payment must be made in full at the time of enrollment.
- Patients are responsible for scheduling visits within the plan year. Missed visits may result in loss of benefits.
- Discounts cannot be combined with insurance or other promotions.
- Excluded from discounts: sedation fees, nitrous, laser tips, cosmetic & specialty lab cases, whitening products, night guards, PerioProtect, and orthodontic clear aligners.
- Preventive and diagnostic procedures are covered 100% under the plan; treatments receive a 10% discount.
